Terms & Conditions: All pre-books placed via email or fax will be verified with a follow-up call based on the
contact information provided on the above form. All pre-booked orders are filled on a first come, first served basis
based upon availability. Pre-booking flu vaccine is by no means a guarantee of delivery. Delivery is completely based
upon manufacturer release of the vaccine to DiaMed USA, LLC. Once the product is delivered, it will be nonreturnable.
Your facility will not be billed until the vaccine is shipped.
